General Terms and Condition
- All services through Alyve Health Platform only
- In Clinic Doctor Consultation – QR Scanning feature applicable wherever UPI is available. Per transaction limit @ INR 1000. All services through Alyve Health Network
- All services applicable to the covered members of the family only and not allowed for any other
- Mid term addition/inclusion allowed only for spouse after marriage / new born baby (intimation within 30 days of the event)
- Mid term Member joining – full premium & benefits applicable for all applicable services
- One single plan applicable for the entire group (selection of plan not permitted)
- Reimbursement for Wallet applicable only if cashless / Alyve Pay is not possible and for the mentioned services as applicable in the plan
- Reimbursement available through Alyve Health application platform only, based on required documents as applicable to every such case
- Children should be under the age of 25 years
- Claim Submission – maximum time limit applicable within 30 days from Bill / Receipt Date
- Waiting period of 7 days since the purchase of the plan
- All documents to be within Plan period only
General Exclusion
- In-patient care and daycare treatments are not covered
- Any unproven procedure/treatment not approved by Indian Medical Council, experimental or alternative treatment including acupressure, magneto therapy etc are not covered
- Alternative medicine systems such as Ayurveda, Yoga and Naturopathy, Unani, Siddha, and Homeopathy are not covered
- Consultations with respect to any nutrition, obesity or cosmetic treatments, or any other treatments which are not medically necessary, are not covered
- Claims arising due to Breach of Law with Criminal Intent, Suicide and Self-Injury etc. are not covered
- Maternity related expenses including general pregnancy, C section, IVF/IUI or any other fertility treatment – Consultation, Injection & medications are not covered
- Insulin injections are not covered
- Syringes and other consumables charges are not covered
- Hair and Acne treatments are not covered
- Medical procedures such as Dialysis, Chemotherapy, Cataract Surgery, LASIK or Plastic surgeries are not covered
- Prosthetics, CPAP machines are not covered
- Claims related to At home care, Day Care & Domiciliary Treatment are not covered
- Over the counter drugs and cosmetic creams or articles, topical medicines are not covered. This exclusion stays with or without a prescription
- Cancer Screening, chemotherapy procedures, drugs to treat cancer are excluded
- Any & all registration charges are excluded from this plan
- AYUSH medicines are not covered
- AYUSH procedures/OPD procedures/physiotherapy/any other day care procedures will not be covered under the plan
